Recovery from illness

There are a number of services available to support you if you are experiencing poor health. They may prevent you from going into hospital, or offer extra care and support after you have been discharged.


reablement

Sometimes people just need a few weeks of extra help at home to get back on their feet, perhaps following a stay in hospital, or after a fall. Having support in place means that it is safe for you to leave hospital. We call this reablement.

If you have been in hospital, you might need some help for a while when it is time to return home. This can help with your recovery and help you become independent sooner.

Sometimes we recognise that you will need a bit more support than can be provided in your home. In these situations we will work with you to determine the best place for you to be whilst you recover.
